In line with Government of India’s vision and flagship scheme of Digital India, Ministry of External Affairs of India has launched eSEWA-an online digital portal for Miscellaneous Consular Services.  The new system would be applicable with immediate effect.  Applicants are requested to apply online for all undermentioned consular services [except for Police Clearance Certificate (PCC) and Emergency Certificate (EC)] as detailed below through the link given below:

Affidavit for Driving license for Indian nationals in Slovakia:

  • Misc Service form duly filled in.  Click here to download the template.
  • Copy of Driving license. 
  • Copy of passport (original to be produced for verification).
  • Copy of Slovak Residence Permit (original to be produced for verification).
  • Fee Euro 9.00
  • Consular surcharge of Euro 2.00 shall be levied on all consular services (Passport, Misc. Services etc.) w.e.f. 01.09.2017.

 

Date of Birth Certificate and any other certificate issued on the basis of valid Indian Passport:

  • One recent passport size photograph. 
  • Copy of passport (original to be produced for verification).
  • Copy of Slovak Residence Permit (original to be produced for verification).
  • Fee Euro 20.00
  • Consular surcharge of Euro 2.00 shall be levied on all consular services (Passport, Misc. Services etc.) w.e.f. 01.09.2017.

 

Power of Attorney attestation:

  • Misc Service form duly filled in. Click here to download the template.
  • One recent colour passport size photographs. 
  • Copy of passport/OCI card (original to be produced for verification).
  • Copy of Slovak Residence Permit (original to be produced for verification)
  • Two copies of Power of Attorney (one original and one copy).
  • Power of Attorney to be signed in presence of Consular Officer.
  • Any witness/witnesses signing on the power of attorney, should also sign in the presence of Consular Officer.
  • Passport/OCI card, Residence Permit of the witness/witnesses (original & copy).
  • Fee Euro 17.00
  • Consular surcharge of Euro 2.00 shall be levied on all consular services (Passport, Misc. Services etc.) w.e.f. 01.09.2017.

Death Certificate/Registration for Indian nationals:

  • One recent colour passport size photograph of the relative/friend who is filling the application form. 
  • Copy of passport/Slovak Residence Permit (original to be produced for verification).
  • Indian passport of the deceased.
  • Death Certificate from local authorities with English translation.
  • No Fees.

 

Attestation of documents related to trade and commerce pertaining to export from Foreign Country to India:

  • Misc Service form duly filled in.  Click here to download the template.
  • One recent colour passport size photographs. 
  • Copy of passport (original to be produced for verification).
  • Copy of Slovak Residence Permit (original to be produced for verification)
  • Original plus Photocopy of the document to be attested.
  • Commercial document should be notarized and apostilled before it is presented to Embassy for attestation.
  • Fee Euro 42.00
  • Consular surcharge of Euro 2.00 shall be levied on all consular services (Passport, Misc. Services etc.) w.e.f. 01.09.2017.

 

Attestation of civil documents (educational certificate/degree, Birth certificate, marriage certificates etc. issued from India):

Please note that the documents should have Apostille stamp on the original document from MEA, New Delhi.

  • Copy of Slovak Residence Permit (original to be produced for verification)
  • Original plus Photocopy of the document to be attested.

Attestation of civil documents (educational certificate/degree, Birth certificate, marriage certificates etc. issued from Slovakia):

Please note that the documents should have Apostille stamp on the original document from Slovakian authorities.

Attestation of pension papers, life certificate pertaining to pensioners:

Affidavit for issue of passport to child living in India:

  • One recent passport size photograph. 
  • Copy of passport (original to be produced for verification).
  • Copy of Slovak Residence Permit (original to be produced for verification).
  • Self-attested copy of passport/marriage certificate of parent in India.
  • Fee Euro 17.00
  • Consular surcharge of Euro 2.00 shall be levied on all consular services (Passport, Misc. Services etc.) w.e.f. 01.09.2017.

Police Clearance Certificate for Indian Nationals:

  • Application form completely filled in online at https://embassy.passportindia.gov.in/
  • One recent colour photographs front profile with white/light background (5 x 5cm). 
  • Visa/Copy of Slovak Residence Permit duly attested by the applicant (original to be produced for verification).
  • Fee Euro 20.00
  • Consular surcharge of Euro 2.00 shall be levied on all consular services (Passport, Misc. Services etc.) w.e.f. 01.09.2017.

 

Police Clearance Certificate for Other than Indian Nationals:

  • Misc Service form duly filled in.  Click here to download the template.
  • One recent colour photographs front profile with white/light background (5 x 5cm). 
  • Copy of passport (original to be produced for verification).
  • Visa/Copy of Slovak Residence Permit duly attested by the applicant (original to be produced for verification).
  • Copies of all visas with entry and exit stamps for the period for which PCC is required.
  • Fee Euro 20.00
  • Consular surcharge of Euro 2.00 shall be levied on all consular services (Passport, Misc. Services etc.) w.e.f. 01.09.2017.

 

Emergency Certificate (One way travel document for India):

  • Misc. Service form duly filled.  Click here to download the template.
  • Two recent colour photographs front profile with white/light background (5 x 5cm). 
  • Original Police report for lost passport along with English translation.
  • Copy of lost passport.
  • Copy of Visa / Slovak Residence Permit (original to be produced for verification).
  • Copy of return flight ticket to India.
  • Fee Euro 12.00
  • Consular surcharge of Euro 2.00 shall be levied on all consular services (Passport, Misc. Services etc.) w.e.f. 01.09.2017

 

Please NOTE:  The Embassy does not apostille any certificate issued in India.  Such appostillations are done within India only through the process of apostille.   The procedure of apostilling/legalizing a document in India is explained at http://mea.gov.in/apostille.htm .  A document apostilled in India does not need attestation by the Embassy.
